St. Clare’s Hospice is a Specialist Palliative Care Unit that delivers a range of services
to our patients and their families. These services include In-Patient stay, Day Hospice,
Consultant–led Out Patient Clinic, Lymphoedema Clinics, Physiotherapy, Chaplaincy
and Complementary Therapies.
In 2014-2015 the Hospice has:

Developed a Befriending Service for socially isolated patients in South
Tyneside with a palliative diagnosis.
•
Recruited and trained a small team of volunteers to deliver the Befriending
Service and a pilot of the service is taking place.
•
Developed a new falls risk assessment tool which is in general use.
•
Our Bereavement Counselling Co-ordinator has enabled us to grow the
service and recruit more volunteers.
•
Introduced a training and development package for the newly recruited
Lymphoedema Specialist Practitioner.
